the availability of properly equipped ambulances -- provided by hospitals rather than by morticians

We might say the byproducts of the morticians' ambulance service were the living patients taken to the hospital -- in marketing terms a "loss leader." Morticians were not inclined to include life support equipment or trained personnel in their ambulances because the majority of their clientele had no need for such services. [AER]
